I confirm that:

• I maintain a physical street address in the state of formation at                       (the registered office), not a P.O. box;
• I am available at that address during normal business hours to accept service of process and official mail;
• I will promptly forward any legal or state documents to the Company; and
• I will notify the Company immediately if I resign or the registered office changes, so the Company can file a change with the Secretary of State.
